Office Hours:
• Mondays: 8.30 am - 4.00 pm
• Tuesdays: 1.00 pm - 6.00 pm
• One Saturday a month: 10.00 am - 2.00 pm
Payments:
We do not accept insurances. However, we will provide you with a "superbill" that can be submitted to your PPO insurance for later reimbursment. All fees for services provided must be paid at the end of the session.
We accept personal checks, Visa and Master cards. Fees will be discussed with an assistant upon making your appointment.
Cancellations:
Any cancellations made less than 24 hours in advance and cancellations of Mondays’ appointments on Fridays will be charged as no show in the amount of appointment fee.

Current patients, non-urgent messages related to medications, forms, etc.:

Calls will be returned within 24 hours on weekdays.
Regular messages left on Fridays will be returned on Mondays.
Refills for medications: ask your pharmacy to fax refill request to 310-821-0060.
• New medications will be prescribed only after office or phone appointment.
• Patients, stabilized on medications will need to come in for a follow up appointment every
three months in order to receive additional refills.
• Prior authorizations, mailing of the prescriptions, letters and forms will be charged
accordingly.
